Every MyCare Concierge membership includes an annual Concierge Physical Examination. It offers the comprehensive approach you might expect from an executive physical at a major medical institution or university health system—but it is conducted by the personal physician who knows you best.

The experience is not a one-size-fits-all checklist. What it consists of may vary based on your age, health history, personal needs, individual risk factors and your physician’s clinical judgment. You and your physician determine what is appropriate, and your physician will explain what to expect.
For Medicare beneficiaries, the Concierge Physical Examination is separate from the Medicare Annual Wellness Visit or services covered by Medicare. All covered medical services will continue to be billed to your insurance as appropriate.
Because each concierge program may include different elements, pricing varies based on the non-covered services your physician includes.

If your preferred physician’s program has reached capacity, our concierge team may suggest another participating MyCare Concierge physician or offer to place you on a waiting list.
Yes. MyCare Concierge is a membership program, not a health insurance plan, and it does not replace Medicare or commercial insurance.
You should keep appropriate coverage for medical care, such as office visits, hospital services, diagnostic testing, specialist care and prescriptions. These covered services will continue to be billed to your insurance as appropriate.
Your annual membership fee is separate from insurance and pays only for the non-covered concierge services and benefits described in your Membership Agreement, including the annual Concierge Physical Examination.
Yes. When appropriate, covered medical services will still be billed to Medicare or your health insurance.
Co-pays, deductibles, coinsurance and other out-of-pocket costs are a legal or contractual obligation, so they still apply according to your health plan.
The membership fee is separate and does not replace or offset any amount you may owe for covered medical care.
Yes. Medicare beneficiaries may participate in MyCare Concierge while continuing to use Medicare for covered medical services.

Your physician’s office will explain whom to contact, which communication methods to use and how medical concerns are handled when the office is closed. Procedures may vary by physician and location.
Each physician’s office maintains procedures for supporting patients when the physician is unavailable. The office will provide information about call coverage, whom to contact and what to expect during planned or unexpected absences.
Coverage arrangements may vary by physician and location. One unique feature of the concierge program is the advance communication you receive about planned physician vacations, allowing you to coordinate any necessary visits ahead of time.
Depending on the physician and location, you may have an opportunity to meet the physician or tour the office before enrolling.
Our concierge team will also explain the program’s services and benefits, membership fee, available payment options and Membership Agreement so you can make an informed decision.
The options available to existing patients depend on the concierge model selected by the physician.
In some models, patients who do not enroll may be able to continue receiving traditional medical care from the same physician. In other models, the practice may help patients understand the available options for establishing care with another physician.
MyCare Concierge staff and the physician’s office will clearly explain what the transition means, whether any action is required and what options are available before you decide.
You are not obligated to enroll simply because you request information or speak with our concierge team.
Before joining, you will have an opportunity to review the program benefits, membership fee, payment options and Membership Agreement.
If you later decide to cancel, MyCare Concierge staff can explain how to submit a written cancellation request and any applicable refund. Your specific cancellation rights and responsibilities are governed by your Membership Agreement.
