MyCare Medical (“MyCare,” “we,” “us,” or “our”), which includes Florida Elite Management, LLC for itself and on behalf of its subsidiaries, affiliates and affiliated physician practices, is committed to protecting the privacy and security of information entrusted to us. This Privacy Notice (“Notice”) explains how we collect, use, disclose, and protect Personal Data and Protected Health Information (“PHI”) when you interact with our website, mobile applications, patient-facing tools, and online services (collectively, the “Services”).

This Notice applies to all users of the Services located in the United States. For information about how we use and disclose PHI under the Health Insurance Portability Act (“HIPAA”), please review our HIPAA Notice of Privacy Practices (“NPP”).

1.Scope of This Privacy Notice

This Notice applies to:

  • Personal Data collected through our public website
  • Personal Data collected through online forms, appointment requests, symptom checkers, and patient-facing tools
  • Personal Data collected through cookies, analytics, and tracking technologies
  • Personal Data collected for marketing, advertising, and website optimization
  • Personal Data collected from individuals located in Texas and Florida

This Notice applies specifically to the www.mycare.us website (the “MyCare Website,” “Website,” or “Site”), and not to any other third-party web sites that may be accessible from here. Therefore, if you are directed to another site outside of this site, MyCare cannot guarantee the privacy practices of those sites meet our standards. MyCare encourages users to read the privacy notices for any site prior to using the site.

This Notice does not apply to PHI collected or used for treatment, payment, or healthcare operations. PHI is governed by our HIPAA NPP, which can be accessed by clicking here on “MyCare HIPAA NPP”.

Health-related information submitted through non-HIPAA forms, precise geolocation (if enabled), biometric identifiers (if used for authentication).

3.How We Collect Personal Data

We collect Personal Data through:

Because our website uses Google Analytics and includes patient-facing tools, certain interactions may involve health-related data that is not PHI but is still protected under federal and state laws, information and data that may be defined as “Personally Identifiable Information” (“PII”) or “Personal Data”.

Request correction of inaccurate Personal Data.

C.Deletion

Request deletion of Personal Data, subject to legal exceptions.

D.Portability

Request a portable copy of Personal Data.

F.Appeal Rights

8.Additional Texas Resident Rights
