The Full Model

In the Full Model, your entire practice transitions to membership-based care. Together, we set a cap on membership, typically between 300 and 500 members rather than several thousand. Every patient gets more meaningful time, more access, and a deeper relationship with you. This model delivers the highest revenue per patient and the greatest reduction in daily patient volume, making it the model of choice for physicians who are ready to commit fully to a concierge model. MyCare Concierge manages the entire conversion process, including a respectful, compliant transition plan for patients who choose not to enroll, so no one is left without a path to continued care. Best for: Physicians ready to build their practice around a defined membership panel from day one.
The Hybrid Model
Two ways to see the same doctor.
The Hybrid Model lets you offer concierge membership to patients who need or want it, while continuing to see your existing fee-for-service and value-based-care patients. You divide your time between the two sides of your practice — concierge members get extended visits, direct access, and priority scheduling, while your traditional practice continues as before. This model is well suited to physicians with a loyal existing patient base who want to introduce a concierge offering without risk. Best for: Physicians who want to test the concierge waters, retain their full existing patient relationships, and grow membership organically over time.
The Transitional Model

The Transitional Model is our most flexible option, and the one most physicians choose when they want the financial and lifestyle benefits of concierge medicine without giving up their existing panel on day one. It’s designed as a bridge — a way to move toward concierge medicine at a pace that fits your practice, with two paths to get there.
Path 1 — Hybrid now, full
concierge later.
Start with the Hybrid Model, seeing both concierge members and traditional patients. As your membership panel fills, we will help you plan and execute a move to the Full Model with a respectful, compliant transition plan for any patients who choose not to enroll.
Path 2 — Care with
an extender.
You focus your personal time and attention on your concierge membership patients, while another provider — working under your supervision — continues to care for patients who haven’t enrolled. Nobody is dismissed from the practice, and your concierge membership grows at its own pace as your total practice revenue grows alongside it.
